One of the most challenging elements (I've found) with game design is that almost any little revision to the overall game-plan, or even the most minor of gameplay alterations, can cause a ripple effect that requires changes across the whole project.
As we attempted to streamline various elements of the game (combat, items, effects, leveling, etc), you can imagine that the inventory that was in place started to become less and less functional. And eventually, it was broken and needed new/revised graphical pieces and a new layout.
